DRUG RECALL - Effective March 6, 2014

On March 6, 2014, Pfizer announced a voluntary, patient-level recall of Effexor XR150mg capsules, NDC 00008-0836-21, Lot # V130142 and NDC 00008-0836-22, Lot #V130140 and Venlafaxine HCI 150mg ER capsules, by Greenstone; NDC 59762-0182-02 Lot #V130014. This recall was initiated because of a pharmacist  report that one bottle of Effexor XR contained one capsule of Tikosyn (dofetilide) 250mcg in addition to Effexor XR capsules.


URGENT PRODUCT RECALL -  Effective February 14, 2014
 
On February 14, 2014 Abbott Diabetes Care initiated a voluntary recall for 2 of their oldest meters FreeStyle Flash and Freestyle Blood Glucose Meters (NDC’s 99073017001 and 99073017001). The reason for the recall is erroneously low results that are not recognized and may pose significant risks. Please note these meters have not been distributed by Abbott Diabetes care since May 2010.
